Breast lifts can be accomplished by several approaches. Sometimes a breast augmentation will literally lift the nipple to a satisfactory point that further surgery isn't desired.

With more droop, a nipple lift is sufficient. This is done by simply removing a crescent shaped area of skin from above the nipple.

In the most advanced cases a standard mastopexy is done, which leaves an anchor-shaped scar which surrounds the nipple and runs down and under the breast. This scar is usually much more acceptable than patients expect.
Here are examples of a standard breast lift and nipple lift. You will notice that the scars are barely visible.
